eldercare case matching for elders and their families. ElderCarelink
assists families in finding a multitude of services, including; assisted living,
nursing homes, adult day care, private duty nursing, care management and homecare,
with participating providers in all 50 states, ElderCarelink identifies
qualified eldercare service providers and product suppliers who meet the specific
needs for each family's individual situation.
ElderCarelink helps families
identify the right type of care and services needed by an aging relative or
friend, highlighting both medical and lifestyle needs, as well as budget and
financing options.

After they determine the needs and preferences of the aging family member,
they are uniquely matched with those participating providers in their network
who can potentially meet their needs. The names of the matched providers are
sent via e-mail to the requesting individual, while the requesting individual's
name and profiling information are passed along to the matched provider(s).
These providers are pre-approved through an authorization on the profile submission
to contact the requesting individual regarding their inquiry and to pursue further
discussions.

All of the participating providers in the ElderCarelink network must
apply to participate and are screened prior to joining the ElderCarelink
network. In addition, each provider must meet ElderCarelink's participation
standards and has contractually agreed to abide by the terms for inclusion in
their network.
ElderCarelink Program Overview
ElderCarelink is a provider referral service that's available to you
at no cost. Their job is to do the research and the
work for you so that you can focus on solving your eldercare needs.
Simply put, ElderCarelink matches you or your loved ones with qualified eldercare service
providers based on some basic information that you make available to them. Your
specific needs are gathered through a brief online survey,
which you complete and submit. The results from your Needs Assessment are then compared against
the provider information in their proprietary database. They will identify the
providers from their network in your particular geographic area which most closely
meet the needs you have identified, and forward them the information that you
provide to ElderCarelink.
Once you submit your Needs Assessment, ElderCarelink will
do the work. Within hours you will receive an e-mail containing information
about the providers that meet your specific needs. You can then expect to be
contacted directly by these providers, who, based on your Needs Assessment,
will be prepared to help you address your eldercare needs. If your particular
situation is urgent, you also have the option of contacting these providers
directly through the contact information that ElderCarelink will provide
you.
